Lipohyalinosis is small vessel disease in the brain. Hypertension is a strong causative factor. So called deep perforating arteries, relatively small arteries branching of from relatively large arteries are especially prone . Normally the tension drops as arteries split into smaller and smaller arteries, but because the small perforating arteries branch directly from larger arteries, they are under relatively large pressure.
